Service users can be referred to us via social workers, however, self-referral can be made by those who are in receipt of personal budgets.

Once we have received your referral, we aim to contact you within 48 hours to discuss the young person’s/adults support needs and ask you to complete our registration form (see above) and return it to New Horizons.
We recommend that you visit New Horizons to discuss the information provided in the registration form in more detail to assist in making the service users time at New Horizons successful. During this visit, any additional forms needed, such as medication or medical needs, can be completed and discussed in more detail.
